**Job Description: Home Care Nurse**
**Responsibilities:**
1. Provide skilled nursing care in patients' homes
2. Assess, develop, and implement care plans
3. Administer medications, monitor vital signs, and perform wound care
4. Educate patients and families on health management
5. Collaborate with physicians and other healthcare professionals
